For in-patient records after DC, out-patient and diagnostic records from UAB Highlands you may fax your request utilizing the approved form from the link below. Make sure to complete the form entirely including that the records being requested are from UAB Highlands. There may be a charge for the release of these records so make sure that section of the form is completed or release of the records could be delayed.
